I'll start working on this but wondered if anyone had seen this before. Sounds like either the permissions have changed on the folder the server uses to store the temp file for upload in, or the size of the file is possibly too large for the settings in the php.ini, or they updated php.ini and forgot to reset the correct folder for uploads (left it at the default location).
