Thanks for your comments! I have considered moving the location of the attachments to under the name. Possibly in the next version

Also... as for the attachment field... let's just say this thing has evolved (an understatement). First of all, if all goes wrong they user can just clear the field. Second, I was having difficult getting javascript to pass the parent/child filed value to a hidden field. Probably another reason in there somewhere... But I must admit, it is a little confusing for someone who has not used it before
