I hope I understand your fix.
You suggest to take out "the new additional "tweak" code,(everything BELOW "my ($UN, $PW) = @_;" EXCEPT: "my ($RowColor, $FieldName, $Notes, $InputField, $Cols, $Rows) = @_;") and put back the old code.
Looking at the MM's instructions,

replace with:
