Mod Name / Version: Notify Moderator Reason 1.0

Description: This adds a field for the user to give a reason when they are notifing
a moderator of a post they should look at. It requires them to give a reason.

Josh... I added this and it's great it works... But one more thing please?

Can you add a version 1.1 where you actually also email the poster's name? I get a lot of situations where one of the Mods deletes the post, but I have no idea who the offender is so I can ban them or delete their account.
