Ok, after much messing around, I got it to work.
It's ugly, but it works fine.
if (($user_title ne "") && (($user_profile[8] eq 'Moderator') || ($user_profile[8] eq 'Administrator') || ((index($user_profile[4],6) > 0)))) {
This ensures all mods, admins and members of forum 6 have their user definable custom title's displayed in all their posts. Good thing I don't have forum 16 or 26 or 36, etc.
