Dumb question time so please forgive as I'm new to this....I want to install who's online hack. I downloaded the whoonline.txt from the database but I've seen references to online.cgi. Is this another file I have to download ?
My ubb is 5.47e

Nope, you have to cut and paste a part from that readme file in a new *.txt file and call it online.cgi.
