That's the best way I could think to explain what I want in a sentence. I want other text boxes for user input. For example, a user would have to fill out "Played by:", "Date played on:", "Location:" text input boxes in addition to the already used "Subject:" text input box. I would like the information that the user inputs to appear in some form in his or her post and on the forum page. I REALLY need this badly, so please someone help me out here. I'm very desperate for this. Also, if someone can make this hack for me, please make it for UBB 6.3. Thanks in advance!

Yes, but I want more than one (like four) text input boxes. Then on the forum page, you would only see the subject column, and then three other columns for the information the users input to the text input boxes. The forum page would look something like this ASCII table:
