I just noticed this in my 5.4 install. For instance, if I search for tattoo...
One of my posts has an inline image with tattoo.jpg (notice the image name is the same as that being searched for). When bringing that post into view (threaded search mode), the image link is broken.
Because the search function italicizes and bolds the search term in the post, it breaks the image link:
http://blah.com/files/[ i ] [ b ]tattoo.jpg[ /i ] [ /b ] as opposed to: http://blah.com/files/tattoo.jpg
Has this been patched up in more recent releases? Or do we need to look into preventing the search engine from picking up search terms within image and other related markup?

Not patched yet. I haven't really been able to find a real solution for this and may need to disable the bolding and italicizing altogether.
